1. Participants to the Career Expedition event are required to accomplish this Event Registration Form or ERF to register for the online event.
2. Check the college programs and colleges/universities that you have indicated above before clicking the SEND button below. Your choice of a college program may be listed in a different dropdown list.
3. The career clinic for the program you will choose in Session 1, 2, 3 or 4 may be dissolved if there are few attendees. You will be given options to attend a different career clinic if the career clinic session you chose gets dissolved.
4. After clicking the SEND button below, you will receive an e-mail confirming your participation to the event. Your preferences among college programs and colleges/universities will also be indicated on the e-mail.
5. If you are a student below 18 years old, you need to ask permission from your parents that you are registering for this online event. This is in conformity to Republic Act 10173 or the Data Privacy Act of 2012. By clicking the SEND button below, you are confirming that you have been allowed by your parents to register for this online event.
6. All personal information you have indicated on this Event Registration Form are held in strict confidentiality and will not be shared with any entity.
7. By clicking the SEND button below, you are affirming that the participant info that you have provided are true and correct, and the participation details you have indicated are your true choices among college programs, careers, and colleges and universities.
