Note: Private schools that want the event conducted exclusively for their Grade 10 students but will not attend the SHS Fair will be required to pay only the speaker's honorarium of P2,500, no Event Management Fee will be required. Further, they may schedule the Choices career webinar at ANY DAY from Monday to Thursday and at ANY TIME. Please indicate this preference under the Notes, Messages, or Inquiry field below.

1. This is just a survey and we know that your preferences may change.

Example, you may choose to join the event FREE OF CHARGE, but later on decide to have it EXCLUSIVELY for your students,

and vice versa. Further, you may indicate that you prefer to join the event in December 2021, but later on decide to join it in

February 2022. We know that some private and DepEd schools are yet to finalize their career programs for this school year.

2. You are not required to pay anything after filling out this form and submitting it. We are just surveying preferences

and possible participants to the event.
