Checklist and notes on the seminar requirements:
1. Confirmation of the webinar at least thirty (30) days before the preferred webinar schedule; if preferred webinar schedule is sooner than thirty (30) days, GuidanceNGO will advise the school or the institution of the availability of the webinar speaker.
2. Honorarium of the speaker (minimum of P2,500 per 45 or 90-minute webinar session) that should be deposited to GuidanceNGO’s bank account. Bank details will be provided thru e-mail.
Note: This is NOT a reservation form, but, a confirmation form. A penalty of PhP1,500 shall be charged to the school per webinar module that is cancelled or postponed. If the cancellation or postponement is due to force majeure or a DepEd directive or an LGU (local government unit) directive, no penalty will be charged to the institution. Please coordinate this matter with the supervising officer/s to avoid miscommunication.
