Checklist and notes on the seminar requirements:
1. Venue (can be dimmed for an effective multimedia presentation)

4. Sound System that can accommodate the sound coming from the laptop
5. Confirmation of the seminar at least thirty (30) days before the preferred seminar schedule; if preferred seminar schedule is sooner than thirty (30) days, GuidanceNGO will advise the school or the institution of the availability of the seminar speaker
6. Honorarium of the speaker (minimum of P3,300 per seminar module for seminars that will be conducted beginning June 1, 2018.) in check payable to Guidance, Inc.
Note: This is NOT a reservation form, but, a confirmation form. A penalty of PhP1,500 shall be charged to the school per seminar module that is cancelled or postponed. If the cancellation or postponement is due to force majeure, no penalty will be charged to the institution.
